"Ozempic Face": Volume First, Not Just Tightening

Ozempic face is the hollow, older-looking face that can follow rapid weight loss. Why speed matters more than the drug, and why volume comes before tightening.

"Ozempic face" is the hollow, older-looking face that can follow rapid weight loss: the fat that gave the cheeks and under-eyes their fullness goes, and the skin above it loses elasticity at the same time. The cause is not the drug itself but the speed at which the weight comes off, so the same face can appear after any diet that moves quickly. And because what is missing is volume, treatment tends to start by restoring volume, with tightening chosen afterwards according to how deep it reaches.

Volume here can mean two things. One is a material placed where the tissue has thinned, such as human acellular dermal matrix (hADM), which gives the skin a scaffold to rebuild on. The other is a collagen biostimulator, an injectable that prompts the skin to lay down its own collagen over the following weeks.

Is "Ozempic face" actually caused by the drug?

No single medication owns this face. Weight that is lost quickly comes off the face as well as the body, and the face has less to spare. The fat pads under the cheeks, temples and eyes thin out, and the skin they used to support is left slightly too large for what is now beneath it.

Two changes overlap here: loss of volume, and loss of elasticity in the skin above. Either one on its own ages a face. Together, and arriving within a few months, they make the change look sudden.

That is why the name is misleading in a useful way. It points at a drug, when the thing to look at is how fast the weight left and which of the two changes is larger for this particular face.

Does tightening help a face that has lost volume?

Tightening treatments heat a layer of the skin so that it produces new collagen and firms up. That addresses laxity, skin that has become loose. It does not put back the fat that the cheeks lost.

On a face whose main change is hollowing, firming the skin over the hollow leaves the hollow where it was. The skin may look a little tighter, and the face still reads as thin.

So when both changes are present, the order usually runs volume first and tightening second. And when tightening does come, the choice is made by the layer a device heats, because the devices differ there, and a face that has recently lost fat is one we usually approach from the dermis.

Why does the depth of a lifting treatment matter?

Lifting devices differ in the layer they heat. Sofwave is a non-invasive ultrasound device whose 7 transducers emit parallel beams that heat the mid-dermis at about 1.5 mm. Its energy does not reach the subcutaneous layer where facial fat sits, so hollowing caused by fat loss is, by mechanism, a different question from what this device works on.

That is why we suggest Sofwave for people with thin faces, or who saw their cheeks hollow after a previous lifting procedure and hesitate about treatments that go deep. Some tightening can be felt on the day, and the visible change from collagen remodelling begins at 4-6 weeks and peaks at around 3 months. How long it lasts varies between individuals.

XERF and LinearZ work in different layers. XERF is monopolar radiofrequency that uses 2 frequencies, 6.78 MHz and 2 MHz, to heat the dermis and the subcutaneous layer beneath it, with one session lasting about 6-12 months. LinearZ is HIFU that delivers focused ultrasound through 4.5 mm, 3.0 mm and 1.5 mm cartridges, reaching from the SMAS fascia up to the dermis, and it is suited to sagging that has begun, a softened jawline and a double chin.

None of this is a ranking. Which device fits depends on which layer needs the heat, and for a face that has recently lost fat, a device that stays in the dermis is the one we tend to reach for first.

What helps restore facial volume after rapid weight loss?

Juveacell is the treatment we consider first where volume and structure have gone. It uses human acellular dermal matrix: donor dermis with the cells removed by a supercritical CO₂ process, leaving the collagen framework intact. Placed in the skin, that framework acts as a scaffold, and your own fibroblasts migrate into it and regenerate there.

Calcium hydroxyapatite (CaHA), poly-L-lactic acid (PLLA) and polynucleotide (PN) each prompt the skin to build its own collagen over weeks, and some of them add a degree of volume on the day through the gel that carries them. Which one is suggested depends on where the volume went and how thin the skin is there, and that is decided in consultation.

How do I choose between volume and tightening?

Stop at the line that sounds like your own face.

  • The cheeks, temples or under-eyes look hollow and the skin is still reasonably firm: volume first, with hADM (Juveacell) or a collagen biostimulator depending on the area.
  • The face looks thin all over, the skin has lost some firmness, and you are wary of anything that goes deep, or your cheeks hollowed after a previous lifting: Sofwave, which stays in the dermis at 1.5 mm.
  • Sagging has begun, the jawline has softened or there is a double chin, and the face still carries its fat: LinearZ for the SMAS, XERF for the dermis and subcutaneous layer, on their own or paired, with LinearZ first and XERF after on the same day.
  • Both hollowing and looseness are there: volume first in the plan, with a tightening treatment chosen by layer, and the two can be done in the same visit with the lifting device going first.

Most people arrive with two of these at once. At the consultation we check fat volume and skin thickness before anything is suggested, because the right sequence changes with what is actually left in the face.

In summary

Rapid weight loss thins the fat pads of the face and loosens the skin over them at the same time. The face looks older for both reasons, and the drug that started the weight loss is the least useful part of the name.

Volume comes first, with hADM or a collagen biostimulator, and tightening comes after. When tightening is chosen, the layer a device heats is the deciding fact, and for a face that has already lost fat we usually start with one that stays in the dermis.

Often yes, and the order on the day is the reverse of the order in the plan. Volume comes first in the plan, but when both are done in one visit the lifting device goes first and the volume treatment is placed after it, so the injectable is not heated. If the injectable was done on an earlier visit, Sofwave is scheduled once about 2 weeks have passed since the injection, and having filler or a scaffold already in place does not rule it out, because its heat stays in the mid-dermis at 1.5 mm. Please tell us in advance what was injected and where.

Juveacell is an injection, so mild swelling for 2-3 days is normal, and with bruising it can run up to 1-2 weeks. Daily life is fine, but allow extra time before an important event. Sofwave has almost no downtime: redness or warmth can appear afterwards and usually subsides within 2-6 hours, and most people return to their day straight away.

Juveacell lasts about 3-6 months with individual variation, and a single session is designed to create the scaffold. Sofwave's collagen change begins at 4-6 weeks and peaks at around 3 months, and maintenance after that is discussed based on your skin. One session of XERF lasts about 6-12 months, and results tend to accumulate with repetition.

Both are performed after numbing cream. Juveacell pain is moderate for full-face injections and mild for the cannula-based under-eye treatment. Sofwave has a real degree of discomfort by the nature of the treatment, and how much of it you feel varies individually.
